As a MassHealth member enrolled with Revere Health Choice, you may be eligible for valuable support with essential needs like food, housing, and a healthier living environment. Our Health-Related Social Needs (HRSN) program is designed to help you overcome barriers and improve your overall health.

What we provide
How HRSN Can Help You
We understand how life's challenges can impact your health. Our HRSN services are here to provide practical, free support in these key areas:
Moving out of homelessness: Help with searching for housing, understanding leases, and moving in.
Keeping Your Home: Support if you're at risk of losing your housing, including landlord mediation and legal referrals.
Making Your Home Healthier: Assistance with improving living conditions related to health needs (e.g., air quality, pest control, cleaning).
Getting Healthy Food: Access to nutritious meals or groceries tailored to your health.
Learning About Nutrition: Guidance from experts on healthy eating habits.
Do You Qualify?
You might be eligible for HRSN services if you are a MassHealth member enrolled with Revere Health Choice and if any of the following statements apply to you:
Homelessness Needs
Are you currently living in a shelter, on the street, or somewhere not meant for human habitation?
Are you 55 or older?
Housing Needs
Are you worried about becoming homeless soon because of a 'Notice to Quit' or 'Eviction Notice'?
Have you been in and out of the hospital multiple times this year?
Healthy Home Needs
Do you have a pulmonary (lung) condition like asthma or COPD, or a cardiac (heart) condition like heart failure or high blood pressure, or a hoarding disorder?
Are you living in conditions that are unsafe or unhealthy due to your medical condition(s)?
Food & Nutrition Needs
Do you worry about food running out before you have money to buy more?
Are you unable to prepare your own meals due to a condition or lack of resources?
Do you have a medical diagnosis, have a behavioral health diagnosis, have a healthy pregnancy, have a high-risk pregnancy, need assistance w/ Activities of Daily Living, and/or go the hospital multiple times?
